Sequel Health 是給自費復健科與疼痛科診所的診後照護作業系統。病人離開診間之後的 90 天，由系統在正確的時間送出正確的內容，收回病人的回報，而每一段內容都經過醫師核准。
A post-visit care operating system for self-pay rehabilitation and pain clinics, built by a practising physiatrist and running on Claude. Every patient-facing module is physician-approved; high-risk questions stop the model and route to a human.

五個工具，每週在台中慈濟醫院復健科門診實際使用
以下不是 Demo，是創辦醫師自己的門診每週在用的工具。全部跑在 Cloudflare Workers 上，直接呼叫 Claude API。
opd-note 病歷產生器
候診問卷加上醫師口述，由 Claude Sonnet 5 草擬 SOAP 病歷。ICD-10 守門員只放行索引裡查得到的診斷碼，模型不可能自己編一個。輸出是能直接貼進醫院 EMR 的純文字，另附一張帶 QR 的 A5 病人說明。

clinic-line 診後追蹤
以 LINE 跑注射療程的 D0 到 D90 追蹤：報到、按日送衛教、收病人回報的功能量表，不必下載 App。2026 年 8 月 29 日上線。

衛教卡專區
142 張由醫師本人撰寫的衛教卡，每張有 QR，病人在診間掃了回家看。病人的提問與回饋回流到資料庫，每月盤點「病人到底在問什麼」，再做成新卡。

醫師貼上病史與理學檢查，Claude Opus 5 從 36 本已數位化的教科書與 PubMed 給出鑑別診斷與下一步，每一句都附書頁或 PMID 可回查。

把一個療程的診後照護，打包成院所自己的 Capability Pack
第一個療程是 PRP 與增生治療。一個 Capability Pack 包含：醫師核准的知識與來源、個人化規則、D0 到 D90 的旅程、警訊停止規則，以及每一段病人內容的真人核准關卡。底層模型可以換，院所累積的能力不會跟著消失。
Packaging each treatment’s post-visit care into a clinic-owned Capability Pack: approved knowledge with sources, personalisation rules, a D0 to D90 journey, safety stop-rules, and a human-approval gate on every patient-facing module. Claude drafts, personalises and triages; clinicians approve.
- Day 1 to 30流程盤點、知識盤點、病人最小資料集、醫師 Persona、警訊規則與基線量測。
- Day 31 to 60Web 與 LINE 沙盒、逐模組覆核、模型路由、三個受限 Agent、代表案例與治理驗收。
- Day 61 to 90在院所核准範圍內小量運作，每週檢視品質、安全、成本與照護結果，交付續約商業案例。

Boundaries · 我們不做的事
AI 不診斷、不開處方、不自己發出任何病人訊息
- 每一段病人看到的內容，都要醫師逐模組核准，不是整批放行。
- 出血、用藥、診斷與治療決策問題命中規則時，模型停止生成，交給真人處理。
- 系統只存病人代號，不存姓名；姓名對照表留在醫師本機。
- 不宣稱已串接醫院 HIS 或電子病歷；病歷仍由醫師確認後貼入。
- 診斷碼只能來自索引，模型不得自行產生。
- Every patient-facing module is approved by a clinician, one module at a time.
- Bleeding, medication, diagnosis and treatment questions stop the model and route to a human.
- Patients are stored as codes, not names; the name map stays on the physician’s own machine.
- No claim of HIS or EMR integration; notes are reviewed by the physician before entry.
- ICD-10 codes come only from our index, never from the model.

台中慈濟醫院復健科醫師，同時具備職能治療師與 ACSM 認證私人教練資格。臨床專長是疼痛治療、增生治療與肌肉骨骼神經超音波，教授超音波導引注射逾三年。每週深度長文與 Podcast 寫給台灣醫師與生醫投資者，發表於 Medium MDBros 專欄。

整套系統由創辦醫師以 Claude Code 建成，跑在 Cloudflare Workers、D1 與 Pages 上，透過 Claude API 使用 Claude Sonnet 5 與 Claude Opus 5。病人端以 LINE 觸及，不要求病人下載任何 App。
